Every form starts from New form at the top of Echo Forms > Forms. It opens Start a new form, with three ways in.
Start from a template
A template is a finished form with real questions, ready to change. Pick the one closest to what you need and press Use this template. It opens in the builder as a new form of your own; changing it never changes the template.
Narrow the list with Search templates… (try quote, intake or waiver) or with the category list, which starts on All categories:

With Echo Forms Signature there are four more categories: Calculators (3), Quizzes (5, each with its answer screens, loading screen and results already built), Surveys (7) and Polls (5). See Quizzes, Survey results and Polls.
A template card that says Some of its fields come with Echo Forms Signature still works without the add-on. The form keeps those fields for when the add-on is on, and leaves them out of what visitors see until then.
Start blank
The first card, Blank form, starts an empty form. Press Start blank, then add your fields from the list on the left. See Build your form.
Describe it and have it built
With Echo Forms Signature and an AI account connected, Describe your form sits at the top of the dialog. Write what the form is for and what it should ask, for example:
A booking request for a dog groomer: name, email, the dog’s size, a preferred day, and anything we should know.
Press Build it. The form is usually ready in under a minute and opens in the builder, unsaved, so you can look over each question before you press Save form.
- It only uses field types this site has, so nothing arrives that you cannot edit.
- It keeps at most 40 questions.
- If it cannot build the form, the dialog opens again with the reason and your description still in the box.
Describing a form needs Echo Forms Signature and an AI account of your own (Anthropic Claude, OpenAI or Google Gemini), connected under Connections > AI. The AI provider bills you directly. See Connections.
Bring a form from another plugin
Moving from Gravity Forms, WPForms, Formidable Forms, Fluent Forms or Contact Form 7? Echo can rebuild the form and bring its entries along. That lives on the forms list, not in this dialog: see The forms list.
